Cancellation Policy
Sometimes, life is hectic & things come up that we cannot anticipate. If you are unable to make a scheduled appointment, I request 24 hours notice, except in the case of illness, personal emergency or unsafe driving conditions.
I am happy to overlook isolated incidents. However, if a client frequently fails to show up for, or is chronically late for their appointments without calling, they will be responsible for the full fee of their scheduled appointment.
Gift Cards
eGift Cards must be printed or downloaded onto your phone & presented at the time of your appointment.
If you do not have a printout or download of your eGift Card, you will need to provide another form of payment.
Plastic Gift Cards must be presented at the time of the appointment.
Gift cards are non-refundable.
Promotions & discounts cannot be applied to gift cards .
If a gift card recipient cancels or misses their scheduled appointment without providing 24 hours notice, the gift card may be considered redeemed except in the case of illness, personal emergency or unsafe driving conditions.
Confidentiality
Any information you share with me is strictly confidential. If you require me to speak with another medical professional with regard to your treatment at Centered Healing Therapeutic Massage, a release form will be provided & there will be no communication without your knowledge and written consent. I do not discuss my clients outside of my practice.
Health History
Whether you are seeking relaxation or therapeutic massage, I am required by AMTA & ABMP to have clients fill out a standard healthy intake before seeing them. As stated above, your personal information is kept strictly confidential. The intake form gives me information that allows me to provide the safe & effective massage therapy to suit your needs.
